Concealed Weapon License Application
You can visit our North Tampa office to apply for a new license, replace a lost license, or renew an existing concealed weapon license.
The Concealed Weapon Intake System (CWIS) allows you to submit your application in person, providing a streamlined process that helps reduce processing times at the Florida Department of Agriculture and Consumer Services (FDACS).
By applying directly at our office, you can:

Additional Concealed Weapon License Services
Note: Your concealed weapon license number is required to complete a replacement or revised license transaction. If you cannot locate your license number, contact the Public Inquiry Center at (850) 245-5691 for help.
Fees: Both replacement and revised license transactions are subject to a fee of $27 ($15 FDACS fee plus $12 convenience fee).
How to Request a Replacement License:
Important: All replacement license requests must include a notarized statement confirming the license is lost, stolen, or destroyed.
You can request a replacement license through three methods:
- Visit one of the Florida Department of Agriculture and Consumer Services (FDACS) regional offices (notarization services are available)
- Appear in person at our North Tampa office
- Mail your request to: Division of Licensing P.O. Box 6387 Tallahassee, FL 32314-6387
How to Request a License Revision:
- If your name has changed, you may request a revised license.
- Name change requests require a copy of a legal document (e.g., state-issued driver license or ID, US Passports or U.S. active/reserve/retired military identification) as proof of the name change.
- Revisions involving name changes will not be printed in a tax collector office. These revisions must be reviewed by FDACS and will be mailed from the state agency in Tallahassee.
- If you need to update only your contact information (address, phone number, or email address), a replacement license is not required. The easiest way to submit updated contact information is through the FDACS website. Click here to access the FDACS website.
- If there is incorrect information on your license (e.g., misspelled name or incorrect date of birth), please contact the FDACS Public Inquiry Center at (850) 245-5691 or chat for assistance.
Photo-Only Transaction:
- Fees: $9 convenience fee (no FDACS fee).
- Applicants visiting our office for a photo-only transaction must bring a copy of their application or online receipt.
- Current concealed weapon license holders may provide their existing license information for a photo-only transaction.
Fingerprint-Only Transaction:
- Fees: $51 ($42 FDACS fee plus $9 convenience fee).
- Applicants visiting our office for a fingerprint-only transaction must bring a copy of their application or online receipt.
- Most applicants are required to submit a full set of fingerprints. Exemptions exist for actively employed and retired Florida law enforcement officers, correctional officers, and correctional probation officers.
